About the Opportunity

We're recruiting on behalf of a respected care provider for an experienced and compassionate Care Coordinator to join their dedicated team.

This is an excellent opportunity for someone with strong leadership skills who is passionate about delivering high-quality, person-centred care. You'll play a key role in ensuring residents receive the highest standard of support while working closely with care staff, healthcare professionals, residents, and their families.

The Role

As a Care Coordinator, you will:

  • Coordinate and oversee the delivery of high-quality, person-centred care.
  • Support residents by ensuring care plans are followed and regularly updated to reflect their individual needs.
  • Organise and lead your shift, providing guidance and support to the wider care team.
  • Administer, record and monitor medication in line with company policies and best practice.
  • Liaise with GPs, nurses and other healthcare professionals to ensure residents receive appropriate care.
  • Maintain accurate electronic care records and documentation.
  • Assist with the induction, mentoring and supervision of new and existing care staff.
  • Promote a safe, caring and respectful environment for residents and colleagues alike.

About You

We're looking for someone who has:

  • Previous experience supervising or leading a care team.
  • A genuine passion for delivering person-centred care.
  • Excellent communication and interpersonal skills.
  • Strong organisational skills with great attention to detail.
  • Confidence using electronic care planning systems.
  • The ability to work flexibly and lead by example.
  • A caring, compassionate and professional approach to supporting residents.

Qualifications

  • NVQ Level 3 in Health & Social Care (or currently working towards it).
